Doulas serving Medi-Cal beneficiaries provide person-centered, culturally competent care that supports the racial, ethnic, linguistic and cultural diversity of beneficiaries while adhering to evidence-based best practices. Doula services are aimed at preventing perinatal complications and improving health outcomes for birthing parents and infants.  Doulas do not provide clinical tasks or replace the medical staff. 

 

​​​​Full-scope Medi-Cal is the same complete coverage you have before or after pregnancy. It automatically includes prenatal care, labor and delivery, and post-partum care for a year after pregnancy

 

MATRESCENCE 

Medi-Cal pregnancy coverage

​The Developmental Transition to Motherhood. A term that describes the physical body, mind, and hormone transition that a woman goes through when she becomes a mother. It encompasses the changes that take place during pregnancy, childbirth and the postpartum period adjacent to adolescence.

THE ROLE OF A FULL SPECTRUM DOULA

​Childbirth assistant is a labor support person a birth worker who provide health education, advocacy, and physical, emotional and nonmedical support for pregnant and postpartum persons before, during and immediately after childbirth (perinatal period) including support during miscarriage, stillbirth and abortion.

THE ROLE OF A POSTPARTUM DOULA

A non-medical assistant during the delicate postpartum period. Supporting birthing people, other caregivers, help face any unexpected challenges.

THE ROLE OF AN END OF LIFE DOULA [EOLD] 

Medi-Cal pregnancy coverage - All other death - Payments due at the beginning of services provided

​A trained end-of-life doula professional provides emotional and physical support services to people who are preparing for or experiencing end of life. Assists a dying person and their loved ones before, during and after death. Opening up conversations about the death.

 

END OF LIFE DOULA - PRE VIGIL 

Medi-Cal pregnancy coverage - for all other death (non Medi-Cal coverage) - $198 - initial 90 minutes   $166 - additional visits​​

The period identified as “Pre-Vigil,” represents the time before the dying person has entered the final days of life, and/or mother with her unborn (neonatal) and for the first year after end of pregnancy.

END OF LIFE DOULA - THE VIGIL 

Medi-Cal pregnancy coverage - for all other death (non Medi-Cal coverage) - $795 - active dying 

The second period, called “The Vigil,” occurs when the dying person is actively dying; Mother laboring period.

END OF LIFE DOULA - POST VIGIL  

Medi-Cal pregnancy coverage - for all other death (non Medi-Cal coverage) - $166 - after death care

The final period, referred to as “Post-Vigil,” occurs after the death.

A life journey – California’s Death with Dignity Law In California, the End of Life Option Act allows terminally ill adults to request medical aid in dying. Here are the key steps and requirements: Eligibility: Patients must be mentally competent adults with a terminal illness expected to result in death within six months. Request Process: Patients must make three requests: two oral requests (at least 48 hours apart) and one written request signed by two witnesses. Consultation: Patients must consult with a consulting physician to confirm their diagnosis and mental competence. To qualify for aid in dying in California, a person must meet the following requirements. Be 18 years or older. Be a resident of California. Be able to self administer the medication.

POSTPARTUM​

The following list of services is intended to indicate the kinds of activities the Doula may perform. Decisions based on appropriateness and Client wishes and will be made in consultation with the Client. Services are broken down into the anticipated periods of service and activities appropriate at those times.

And let's talk about postpartum: A Developing Response

​

Postpartum is not a single moment it is an ongoing process. After birth, a mother's body, mind, and identity are all adjusting simultaneously. Hormones shift, sleep is interrupted, emotions ebb and flow, and relationships are renegotiated. this period is a developing response to the profound changes of birth, requiring time, support, and gentle care. 

​

A doula's role during postpartum is to: 

  • Provide emotional support as you navigate new challenges. 

  • Offer practical guidance for recovery, infant care, and daily life. 

  • Help you honor your own needs while caring for your baby. 

  • Recognize the subtle shifts in your identity, supporting the ongoing transformation into motherhood. 

  • ​With attentive care, postpartum can be experienced as a journey of growth, connection, and self-discovery.

Together, this work reflects the heart of Compassionate Responders: creating steady, non-clinical spaces where people are met with dignity, regulation, and humanity. You do not have to do this alone. Creative Conversations - artistic activities, supplies included.

This 9 phase life survey journey ensures

  • Identity is never shamed

  • Beliefs are honored

  • Change is body led, not forced

  • Worth is inherent, not earned

  • Growth stays within the window of tolerance

Each phase builds upon the previous, creating a life survey - a visual representation of personal growth through the nine phases. Participants can track insights, emotional patterns, and evolving values, producing a holistic picture of inner landscape.

LIMITATIONS
the Doula is not a replacement for medical care. The Doula will not give medication. If the Client can't administer medication safely for any reason, the Client understands that the Doula will notify the prescribing doctor or appropriate member of the medical care team, so a solution to the problem can be found. 
When the Doula is acting in the capacity of a doula and under terms of this agreement, she will not act as a licensed clinician (such as a nurse or social worker).
Please note that while the Doula will make every effort to be present at the time of need, or provide coverage, this is not always feasible. 
upon request, the Doula will help the Client consider the pros and cons of a decision they are unsure about, but will not make decisions for the Client, nor promote a particular course of action. 

 

PLACE OF SERVICE
The Doula will provide services in the Client's home and/or a facility setting where the Client may be placed for temporary care, respite for the caregiver. If the Client is in a facility, the Doula will abide by all the rules and regulations of that facility. 
If the Client is in a facility the Doula will make every effort to honor the spirit of the plan. 


RESPONSIBILITY OF THE CLIENT
The Client will inform the Doula of any health conditions or complications the Doula should know about what could impact the health of the Doula or dramatically affect the care expected of the Doula. 
The Client will keep the Doula apprised of any substantial changes in treatment or medication. 
The Client will inform the Doula of any environmental conditions, such as active smoking in the home when oxygen is in use, potential allergens, or infestations of pests that could impact the Doula or might limit the Doula's ability to provide services. 
The Client will inform their medical care provider that they have hired the Doula and get permission to pass along to the Doula all useful contact information. 
If the Client, or an official representative who has the legal right to speak for the Client, decides at any point to the end the services of the Doula after having signed the Agreement, they will notify the Doula as soon as possible.

WAIVER OF LIABILITY
As the express condition of the Doula's performance of services, the Client agrees, on their own behalf and on behalf of all persons or entities that may be bound under the law of the state of California to waive any rights, claims or causes of action that the Client may have against the Doula, now and in the future, with respect to the death of the person the Doula was hired to serve, or with respect to the services the Doula may provide to the Client. The Client expressly understands that the Doula is not a medical professional and therefore cannot and will not provide the Client with medical advice.

Key points about Medi-Cal coverage in California
Statewide program: Medi-Cal is a statewide program that ensures access to healthcare services for low-income Californians. 

Eligibility based on needs: Eligibility is primarily based on income and family size, with various other factors like, age disability status, pregnancy, and certain health conditions considered.  

No distinction based on incorporated vs. unincorporated status: your location within a county, whether it's incorporated city or an unincorporated area, does not affect your eligibility for Medi-Cal. Central California Alliance for Health coverage areas.
